Client Satisfaction Survey FAQs

The survey will automatically be sent to the email address and/or phone number on file in your e2Fulton record after you receive a service from one of our funded agencies. The survey will be sent automatically via email or text; you cannot fill out the survey by scanning a QR code. Surveys are random and will not be sent out for every service.
